Snorkeling Adventure Questionnaire

Before we complete your booking please read and fill out the following form.

** Please note that snorkeling and scuba diving are strenuous activities. They are done in the ocean, which is subject to currents and waves. We do shore dives and snorkeling experience. Also be aware that Hawaii does not have lifeguards at all beaches. **

*Minimum age for snorkeling is 5 years old*

What would you say that your swimming level is? (please read options carefully)

-Non-swimmer or Beginner (may show paddling and kicking coordination but uses assistance when swimming, may demonstrate a fear of the water)

- Intermediate (Demonstrates forward motion in water on front and back without assistance. Able to swim without touching the ground. Comfortable in the water. Does NOT have fear of the water)

- Advanced (Has experience swimming in the ocean with waves and current. Comfortable in the ocean swimming without touching the ground for at least 10 minutes. Able to dive to the bottom of the ocean.)
